LIABILITY WAIVER AND MEDIA RELEASE FORM

ASSUMPTION OF RISK AND WAIVER OF LIABILITY

I acknowledge that participation in cheerleading, tumbling, and related activities involves inherent
risks of injury, including but not limited to sprains, fractures, paralysis, or even death. I understand
that these risks cannot be eliminated entirely regardless of the care taken to avoid injuries.

I hereby voluntarily assume all risks associated with participation in any event, training, competition,
or activity hosted by Big Beach Tumble + Cheer and/or the Alabama Gulf Coast Sports Center (AGC
Sports Center).

I agree to release, waive, discharge, and hold harmless Big Beach Tumble + Cheer, AGC Sports
Center, its owners, directors, staff, volunteers, and affiliates from any and all liability, claims,
demands, or causes of action, now and in the future, for personal injury, property damage, or
wrongful death arising from participation in its programs.

MEDICAL EMERGENCY CONSENT

In the event of a medical emergency, I authorize staff or volunteers of Big Beach Tumble + Cheer or
AGC Sports Center to secure medical treatment for the participant. I understand that every effort will
be made to contact the parent/guardian in such situations. I assume all financial responsibility for
any medical treatment rendered.

PHOTO, VIDEO, AND SOCIAL MEDIA RELEASE

I hereby grant permission for Big Beach Tumble + Cheer and AGC Sports Center to take
photographs and/or videos of the participant during activities and events. I understand that these
images may be used in promotional materials, including but not limited to social media, websites,
flyers, and digital marketing.

I waive any rights to compensation or ownership of such images and release the organizations from
liability in connection with the use of these media materials.

ACKNOWLEDGEMENT AND SIGNATURE

I have read and fully understand this waiver and release of liability and sign it voluntarily. I certify
that I am the parent or legal guardian of the participant (if under 18), or that I am the participant and
18 years of age or older.
